| INDIVIDUAL APP REQUIREMENTS | $300 certificate fee and $50 app fee |
| INDIVIDUAL EXAM FEES | Third party exam. $50 for each attempt. |
| COMPANY APP REQUIREMENTS | $250 every 2 years |
| REFRESHER TRAINING | All licenses must be renewed every 2 years by successfully completing refresher training |
| RECIPROCITY | An applicant shall be considered for certification by the department under an equivalent certification agreement established at KRS 211.9069. |
